Rivier Athletics Medical Clearance Information

NASHUA, N.H. – All Rivier students attempting to participate in athletics during the 2024-25 academic calendar must provide the Sports Medicine department with several pieces of information, including current address, emergency contact, health insurance, medical alerts, and health history information.

All of this information will be entered and stored in our medical records system called SportsWareOnLine (SWOL) by the deadline of August 1, 2024. Failure to complete the appropriate forms by the deadline will preclude you from participating.
