Medical Clearance
Prior to participating, all athletes must provide the Sports Medicine Department with several pieces of information, including current address, emergency contact, health insurance, medical alerts, and health history information. All of this information will be entered and stored in our medical records system called SportsWare OnLine (SWOL).

Returning Student-Athlete Checklist:
-Returners do not need new physicals.
-Any major medical updates should be accompanied by a written letter from your physician and updated on your medical history.
Printable Directions & Health Forms:
1. Pre-Participation Physical Exam Directions/Form
Your PPE form must be signed by your Physician and uploaded into SWOL.
2. Sickle Cell Results/Directions
Your Sickle Cell results from your Physician must be uploaded into SWOL.
****FAILURE TO COMPLETE ALL OF THE APPROPRIATE FORMS by 8/1 WILL PRECLUDE YOU FROM PARTICIPATION IN ATHLETICS AT RIVIER UNIVERSITY UNTIL THEY ARE COMPLETED.****
